Requirements

• Bachelor's degree in Social Work required.
• Certified Case Manager credentialed from the Commission for Case Manager Certification (CCMC) preferred. ACM also accepted.
• Certified Social Worker preferred.

Nice To Haves

• Masters Degree in Social Work
• LCSW or LMSW
• Acute Case Management preferred

Responsibilities

• Oversee complex discharge plans, ensuring patients have crucial resources at home, such as housing, follow-up care, and medications.
• Tackle non-medical issues and address the root causes: referred to as Social Determinants of Health (SDOH)—like food insecurity, housing instability, and financial challenges that affect patients' ability to heal and adhere to treatment.
• Deliver vital mental health counseling, offer crisis intervention, and connect patients to behavioral health services.
• Serve as a strong advocate for patients and families, coordinating various community resources to improve the patient's overall well-being post-discharge.
