About The Position

Pomeroy Living is seeking a compassionate and experienced Social Worker to join our team. As a key member of our interdisciplinary care team, you will play a vital role in providing psychosocial support and advocacy to our residents.

Requirements

Minimum of Bachelor’s Degree in Social Work or a bachelor’s degree in a human services field including, but not limited to, sociology, gerontology, special education, rehabilitation counseling, and psychology, Master’s Degree preferred.
One year supervised social work experience working directly with individuals in a long-term or sub-acute care rehabilitation.
Excellent communication, interpersonal, and documentation skills.
Strong assessment, problem-solving, and critical thinking skills.
Ability to work independently and as part of a multidisciplinary team.
Passion for working with older adults and a commitment to providing high-quality care.

Nice To Haves

Current Social Work license in the state of Michigan preferred

Responsibilities

Conduct comprehensive psychosocial assessments of residents, including identifying social, emotional, and spiritual needs.
Develop and implement individualized care plans that address resident needs and support their overall well-being.
Provide counseling and support to residents and their families on a variety of issues, such as grief and loss, adjustment to care, and end-of-life care.
Advocate for resident rights and ensure their quality of life.
Assist with discharge planning and coordinate community resources.
Collaborate with the interdisciplinary care team, including physicians, nurses, therapists, and other staff.
Maintain accurate and timely documentation in accordance with regulatory requirements.
Participate in facility-wide quality improvement initiatives.
Conduct resident and family meetings as needed.
Provide support and guidance to staff on psychosocial issues.
