About The Position

The Social Worker assesses the psychosocial needs of patients and families and identifies appropriate interventions to support those needs. The role may provide crisis intervention and supportive counseling to patients and families throughout the hospital. The Social Worker advocates for patients and families and provides counseling and support for those dealing with chronic illness and new-onset diagnoses. The role also provides care coordination, refers patients and families to hospital support services and appropriate community-based organizations, and collaborates with medical providers as a member of the multidisciplinary team.

Requirements

• Master’s degree from a school of social work accredited by the CSWE.
• Complete and maintain all competencies and trainings required for the role.

Responsibilities

• Assess patients’ and families’ psychosocial needs and identify appropriate interventions.
• Provide crisis intervention and supportive counseling to patients and families in all areas of the hospital.
• Advocate for patients and families.
• Provide counseling and support to patients and families dealing with chronic illness and new-onset diagnoses.
• Provide care coordination services to patients and families.
• Refer patients and families to hospital support services and appropriate community-based organizations.
• Collaborate with patients’ medical providers as a member of the multidisciplinary team.
