This role provides supportive services and counseling on healthcare and home care programs and services. The Social Worker serves as a member of the interdisciplinary team, assisting with social, emotional, and economic concerns of patients, clients, residents, and their families or caregivers to help them achieve or maintain an optimal level of functioning by coordinating and planning programs. The position involves crisis intervention, assisting families in understanding medical situations, and coordinating healthcare programs among various stakeholders. The Social Worker demonstrates knowledge of human behavior, responds to suspected abuse, neglect, or violence, and develops appropriate plans of care by utilizing resources from social, health, and human services agencies. They provide referrals, information, and education, and demonstrate commitment to the organization through effective time utilization, participation in special projects, and flexibility. Professionalism is demonstrated through care conferences, transitional rounding, and advocacy. The role requires critical thinking, problem-solving, decision-making, and strong written and verbal communication skills to establish rapport with patients, families, and healthcare teams. Independent judgment is expected, and active participation with healthcare teams is essential. Depending on the department, responsibilities may include providing social services for donors and transplant recipients.
