About The Position

At CareOne, our Social Workers play a vital role in supporting residents, families, and the interdisciplinary care team to ensure the highest level of emotional, psychosocial, and discharge planning support throughout the resident stay. As a Social Worker with CareOne, you’ll work collaboratively with nursing, rehabilitation, admissions, and leadership teams to help residents navigate complex healthcare needs while maintaining dignity, independence, and quality of life.

Requirements

  • Master’s Degree in Social Work or related human services field.
  • Current CSW or LSW licensure within the state of operations.
  • Strong communication, organizational, and interpersonal skills.
  • Ability to work collaboratively within an interdisciplinary healthcare environment.

Nice To Haves

  • Minimum of one year of Social Work experience preferred.
  • Long-term care or skilled nursing experience preferred.

Responsibilities

  • Participates in an interdisciplinary team approach to support resident care, discharge planning, and overall quality outcomes.
  • Provides therapeutic intervention and emotional support to residents and families coping with illness, disability, institutionalization, or life changes.
  • Completes psychosocial assessments and maintains timely, accurate documentation in accordance with company policy and regulatory requirements.
  • Develops and implements individualized social service care plans with measurable goals and interventions.
  • Participates in interdisciplinary care plan meetings and ensures resident and family involvement in the care planning process.
  • Acts as a liaison between residents, families, facility leadership, and outside agencies to support resident rights and advocacy.
  • Conducts family meetings, support meetings, and other resident-centered communication initiatives as needed.
  • Facilitates discharge planning and coordination of services to support safe and successful transitions of care.
  • Assists with admissions processes including referral coordination, insurance verification, and facility tours when needed.
  • Develops and maintains positive relationships with referral sources, healthcare partners, and community organizations.
  • Supports a safe environment for residents, visitors, and staff while following all center policies and procedures.
