About The Position

Vi at Bentley Village is seeking a Full-Time Social Worker for their skilled nursing facility. The Social Worker will provide medical social services to residents, assessing their social, financial, and emotional needs. This role involves making appropriate referrals, resolving problems, and facilitating case management for resident transitions within the community's continuum of care. The position operates under the direction and supervision of the community's Social Services Manager.

Requirements

Must have experience working in a skilled nursing facility.
2 years related experience with at least 1 year of experience in a health care setting (hospital, clinic, rehabilitation center, etc.).
Bachelor's Degree in Social Work, Psychology, Gerontology or Sociology from an accredited institution.
Excellent problem solving skills, using a customer-focused approach to problem solving and goal setting.
Excellent communication and listening skills.
Enthusiastic, resourceful, organized and possesses strong personal awareness of nonverbal communication.
Computer skills including proficiency in MS Office.
Compliance with Florida’s Care Provider Background Screening process.

Responsibilities

Provides medical social services to residents in accordance with physician orders and under the direction and supervision of the community's Social Services Manager.
Assesses residents' social, financial and emotional needs.
Makes appropriate referrals for resolution of problems.
Facilitates aspects of case management regarding resident transitions through the continuum of care in the community.
