Social Worker - Women & Children (Ambulatory Care)
About The Position
Social Work Services at the Mount Sinai Hospital is a comprehensive department serving a diverse patient population. The department emphasizes the physical, emotional, and social needs of each patient and their care partners, collaborating within interprofessional teams. Founded in 1907, it is one of the oldest hospital social work departments in the nation, employing over 450 licensed social workers across more than 65 program/service areas. Employment offers a pathway to LCSW licensure and access to continuing education and professional development. This grant-funded position will provide complex care management and comprehensive social work services to children and their families, with a mission to provide access to social work services for the underserved.
Requirements
- NYS LMSW/LCSW.
- Demonstrated clinical skills, preferably with children and adolescents within ambulatory care populations.
- Strong teamwork and organizational skills.
- Ability to collaborate with a large group of medical providers.
- Ability to work with a large population with variable social work needs for interventions.
- Minimum of one year in current position unless a Float in Women’s and Children’s.
Nice To Haves
- Bilingual (English/Spanish) preferred.
Responsibilities
- Provide primary mental health evaluations and short-term treatment.
- Offer complex care management and collaboration with community providers.
- Provide supportive counseling and psychoeducation to encourage adherence to primary care and adoption of family-centered wellness behaviors.
- Follow families at high risk for problematic parenting.
- Encounter families at risk of abuse/child maltreatment and collaborate closely with the child protection social worker.
- Collaborate with attending physicians, resident physicians, nurses, and administrators as an integral part of the pediatric practice.
- Work flexibly and set priorities in a fast-paced setting.
- Engage families during pediatric appointments and work outside the confines of an appointment schedule.
- Maintain data on cases served as a key component for program metrics and deliverable services.
