About The Position

Social Work Services at the Mount Sinai Hospital is seeking a Social Worker (0.6 FTE) to provide psychosocial services, including discharge planning, for hospitalized stroke patients on 8W/8C, as well as other patient populations with neurologically-based illnesses and surgery. The Mount Sinai Hospital is a 1,134-bed facility with an extensive outpatient and specialty care network and is the largest hospital in the Mount Sinai Health Care System. It offers comprehensive social work services and programming to meet the myriad needs of the diverse populations we serve. We believe that total patient care must emphasize the physical, emotional, and social needs of each patient and their care partners. Social Workers collaborate within interprofessional teams to serve patients and the larger community from both a micro and macro level including both direct care and prevention. Founded in 1907, the Department of Social Work Services at the Mount Sinai Hospital is one of the oldest hospital social work departments in the nation with over 450 licensed social workers employed across more than 65 different program/service areas. Employment in our department provides a pathway to LCSW licensure as well as access to a broad range of continuing education, professional development and extra-curricular opportunities.

Requirements

  • NYS LMSW or LCSW.
  • Excellent organizational, collaborative, communication and short-term psychosocial assessment and intervention skills.
  • Ability to develop and implement safe, appropriate, and timely discharge plans in a demanding, fast-paced environment.
  • Ability to coordinate resources within hospital and community networks for effective management of patient care.

Nice To Haves

  • Inpatient hospital experience preferred.
  • Experience with or interest in grief/trauma/terminal illness.
  • Experience with or interest in crisis intervention work.
  • Experience with or interest in program development/research opportunities.
  • Internal applicants: Minimum one year in current position unless currently a Float in Medicine/Surgery.

Responsibilities

  • Provide psychosocial services, including discharge planning, for hospitalized stroke patients on 8W/8C as well as other patient populations including neurologically-based illnesses and surgery.
  • Conduct a comprehensive assessment of stroke patients, including depression screening using a standardized screening tool.
  • Collaborate with the interdisciplinary team in support of the Comprehensive Stroke Center program.
  • Be actively involved in the Respiratory Recovery Pathway.
  • Opportunity to do program development, e.g., Performance Improvement projects and group work.
  • Coverage includes 8W and other medicine/surgery units.
