About The Position

The Lundquist Institute for Biomedical Innovation at Harbor-UCLA Medical Center is seeking a Social Worker Site Coordinator for its Pediatrics & Prenatal Sites. This role involves providing professional clinical social work services to patients and families facing complex medical and psychosocial challenges. The coordinator will assist patients in resolving social and structural barriers to health, facilitate effective use of healthcare and community services, and perform social work services related to the coordination of care for clients needing psychosocial, mental health, and economic stability support, with a focus on preventing and addressing childhood adversity. The position is based at the Torrance Campus.

Requirements

  • Master's Degree in Social Work from a graduate school accredited by the Council on Social Work Education, including supervised clinical social work field work experience.
  • Valid Associate Clinical Social Worker (ACSW) registration issued by the California Department of Consumer Affairs Board of Behavioral Sciences, or intention to obtain.
  • One year of experience in medical, mental health, child- and family-facing services, or child welfare social work setting(s) involving problems of medical, mental, or emotional adjustment in a medical setting (if Option II is chosen).
  • Valid California Class C Driver License or ability to utilize an alternative method of transportation.

Nice To Haves

  • At least 2 years post-degree experience working in a multidisciplinary team within a medical setting assessing psychosocial needs of patients and families.
  • At least 2 years post-degree experience evaluating infant, family, early childhood mental health and developing interventions/care plans.
  • Ability to work in a fast-paced environment with minimum supervision.
  • Spanish language fluency.
  • Previous experience supervising and/or managing individuals or teams, ideally MSWs or MSW interns/students.

Responsibilities

  • Delivers longitudinal social risk/need resource coordination services, including Medical-Financial Partnership clinical, treatment, and support activities in prenatal, family medicine, and/or pediatric clinic settings.
  • Supports connection to psychosocial, mental health, and economic stability supports from prenatal, perinatal/nursery, pediatrics, and/or family medicine care, as well as community supports.
  • Supports and supervises Medical-Financial Partnership team members, including MSW trainees.
  • Collaborates and coordinates with healthcare team members, including trainees, LAC-DHS social workers, and community service providers to address adverse childhood experiences and identified psychosocial, mental health, and economic risks/needs.
  • Provides direct Medical-Financial Partnership family resilience coaching services to patients and caregivers.
  • Conducts interviews, assessments, and evaluations to identify social, emotional, and economic barriers, needs, and strengths, and factors interfering with accessing healthcare and community services.
  • Identifies, refers to, coordinates, and monitors community resources, agencies, other professional disciplines, and County departments to provide services according to the patient treatment plan.
  • Documents social/emotional assessments, progress, and treatment plans in accordance with utilization review standards and medical record requirements, communicating with the primary care team.
  • Develops and implements social work treatment plans in consultation with the Medical-Financial Partnership team and clinical social work team, in collaboration with patients and families.
  • Participates in multi-disciplinary health team conferences to formulate, assess, and consult on patient/family care plans, interpreting psychosocial and economic aspects.
  • Participates in multi-disciplinary orientation sessions and in-service training for other disciplines to raise awareness of psychosocial risks and needs.
  • Attends in-service training, continuing professional education, seminars, and workshops for ongoing professional development and to meet California state licensure requirements.
  • Manages coordination and monitoring of daily calendars, team meetings, and visits for Medical-Financial Partnership team members and clinical encounters.
  • Facilitates daily team huddles and additional team meetings, manages the Medical-Financial Partnership calendar, and supervises the team’s daily care coordination and service responsibilities.
  • Provides performance feedback and support to reporting staff, ensures fidelity of Medical-Financial Partnership services, supports service delivery through quality improvement and assurance activities, and participates in program implementation and staff training evaluation.
  • Confers with program directors, managers, and supervisors regarding human resources, program resources, work problems, and opportunities for program effectiveness.
  • Implements clinical workflows for family strengths and needs identification and response in collaboration with supervisors, directors, team, and providers.
  • Conducts quality-assurance activities and carries out policies and procedures.
  • Engages in clinical quality improvement activities to enhance team effectiveness.
  • Stays informed on early relational health, socio-emotional development, trauma-informed care, evidence-based practices, integrated healthcare models, quality improvement, law and ethics in social work, and the local community resource landscape.
  • Participates in Medical-Financial Partnership multi-disciplinary team conferences to discuss and evaluate client progress and develop care coordination and transition plans.
