About The Position

At Corewell Health, the Care Management teams are over 1,000 individuals strong, composed of various specialists working across different settings to provide quality patient care. Social Workers with LMSW or LLMSW are integral to this team, offering patient-centered care and acting as problem-solvers, detectives, and advocates. They connect patients to necessary services, support end-of-life journeys, manage transitions in care, provide mental health support, and offer education and resources for specific diagnoses. The Care Management Teams focus on whole-person care, addressing social factors that impact a patient's overall wellness.

Requirements

• LMSW - Master Social Worker - State of Michigan License Upon Hire required
• Master's Degree in Social Work
• 2 years of relevant experience

Responsibilities

• Provide quality patient care under the direction of the Care Management Leadership team.
• Recognize the impact of care and partnerships with patients/families to reach high-quality patient outcomes.
• Drive patient-centered care forward as key contributors on the interdisciplinary team.
• Ensure the patient’s voice is heard and their needs are met.
• Act as master problem-solvers, detectives, and advocates, connecting patients to necessary services.
• Provide expert care in support of an end-of-life journey.
• Manage significant transitions from home to the next level of care.
• Provide therapy to support a mental health journey or life change.
• Offer special attention to a certain diagnosis that needs education, support, and resources to improve a patient's overall health outcome.
• Focus on whole person care, paying special attention to social factors that are impacting a patient’s overall wellness.
