About The Position

Provide case management, care coordination, and long-term care screenings to Rice County residents. This role involves working as part of a long-term care team at Rice County Public Health, assessing and evaluating clients' health and living situations to determine the level of care and services needed. The position also includes providing relocation assistance and coordinating client care with various stakeholders.

Requirements

• Bachelor’s degree in Social Work or related human services field, or equivalent.
• Valid Driver’s license is required upon hire.

Nice To Haves

• One year of social work experience in home or community-based services.
• Experience conducting MNChoices assessments and/or waiver case management services.
• Bilingual: (English/Spanish and/or English/Somali).

Responsibilities

• Provide long-term care consultation services in home and community settings.
• Assess and evaluate clients' health and living situations to determine level of care and services needed.
• Provide care coordination and case management services.
• Provide relocation assistance including assessment, community support plan development, and referrals as needed.
• Coordinate client care with medical providers, vendors, clients, and families.
• Maintain accurate and up-to-date records.
• Other duties as assigned.
