About The Position

Social Work Services at the Mount Sinai Hospital is a comprehensive department serving diverse populations with a focus on the physical, emotional, and social needs of patients and their care partners. Social Workers collaborate within interprofessional teams, engaging in both direct care and prevention. The Mount Sinai Adolescent Health Center (MSAHC) aims to eliminate barriers to healthcare for young people by providing high-quality, comprehensive, integrated, and confidential services at a low cost. MSAHC is a leading center for clinical care, specialized training, research, and advocacy in adolescent and young adult health. The A28 Social Worker will provide social work services to at-risk adolescents aged 10-26 and their families within an integrated medical and mental health primary care setting at MSAHC. Responsibilities include short-term individual, family, and group psychotherapy, intensive case management, crisis intervention, and health education as part of an interdisciplinary team. The A28 SW will consult with medical providers on high-risk patients with complex medical and mental health needs and conduct clinical assessments. A key focus will be developing innovative models to engage and retain adolescents with psychosocial needs, particularly those with a history of trauma.

Requirements

NYS LCSW or LMSW

Significant experience working with low-income, inner-city, HIV positive, at-risk adolescents.
Skillful in assessing patient needs in relationship to health and psychosocial functioning and establishing intervention accordingly.
Experience identifying and collaborating with external resources (e.g. housing, public assistance, insurance, schools) and other specific services relating to HIV positive and High Risk young adults required.
Experience in delivering therapeutic interventions, such as preventative and risk-reduction models that reflect developmentally appropriate methods and materials in working with adolescents and young adults (i.e. TF-CBT, CBT, MI, DBT, EMDR, CPT, IPT-A).
Experience in providing individual therapy and group work.
Excellent collaborative, clinical and communication skills.
Excellent organizational and documentation skills.
Working knowledge of state regulations related to child abuse, neglect, and other urgent care issues.

Nice To Haves

Experience working with populations with complex trauma histories preferred.
Experience working with LGBTQIA identified adolescents and young adults preferred.
Ability to be flexible and use non traditional approaches in working with young adults preferred.
Bilingual (English/Spanish) preferred.

Responsibilities

Provide social work services to high-risk adolescents who are HIV positive and who are high risk for contracting HIV, and their families at AHC in the context of a team approach-Project IMPACT.
Provide intense Medical case management, crisis intervention, short-term and ongoing individual, family, and group supportive counseling services, as part of an inter-disciplinary supportive case management and medical team.
Participate in developing strategies to recruit and retain adolescents who are at high risk for HIV and those who are HIV positive in care.
