Requirements

Master’s degree in Social Work
Current Licensure in the State of Kansas
Minimum two year’s work experience in the mental health field
Knowledge of third party payers including but not limited to various types of policy coverage

Nice To Haves

A practicum in acute mental health
Current Basic Life Support for Healthcare Providers (BLS)

Responsibilities

Provide psychosocial services, including patient/family counseling and crisis intervention during hospitalization.
Conduct comprehensive discharge planning and coordinate community resources.
Assist patients and families in the decision-making process.
Collaborate with Case Management, Social Services, and other healthcare providers to evaluate patient conditions.
Evaluate patient needs and counsel them on overcoming conditions and avoiding dependencies during recuperation.
Evaluate and suggest appropriate in-home help for newly discharged patients.
Work with the multidisciplinary treatment team to provide consultations.
Provide staff development and training on assessments, reassessments, care plan development, interventions, prioritizing needs, case conferencing, and follow-up.
Conduct Psycho/Social problem assessments.
Perform other duties as needed or assigned.
