About The Position

Social Work Services at the Mount Sinai Hospital is a large department with over 450 licensed social workers across more than 65 program/service areas. The Mount Sinai Adolescent Health Center (MSAHC) provides comprehensive, integrated, and confidential services to young people at no cost, aiming to break down barriers to healthcare and wellness. The A28 Social Worker will provide social work services to at-risk adolescents and their families in an integrated medical and mental health primary care setting at MSAHC. This role involves short-term individual, family, and group psychotherapy, case management, crisis intervention, and health education. The social worker will consult with medical providers, conduct clinical assessments, and participate in developing innovative models to engage and retain adolescents with psychosocial needs, particularly those with a history of trauma.

Requirements

  • Licensed social worker
  • Experience providing social work services in a primary care setting
  • Experience working with adolescents and their families
  • Experience in individual, family, and group psychotherapy
  • Experience in case management
  • Experience in crisis intervention
  • Experience in health education
  • Ability to consult with medical providers
  • Ability to conduct clinical assessments
  • Experience working with individuals with a history of trauma (e.g. sexual abuse, domestic violence, community violence, physical abuse)

Responsibilities

  • Provide social work services to at-risk adolescents and their families in an integrated medical and mental health primary care setting.
  • Provide short-term individual, family, and group psychotherapy.
  • Provide intense case management.
  • Provide crisis intervention.
  • Provide health education as part of an interdisciplinary medical and mental health service model.
  • Consult with medical providers working with high-risk patients with medical and mental health needs.
  • Conduct clinical assessments on patients referred by medical providers.
  • Participate in developing an innovative model to engage and retain adolescents who have a variety of psychosocial needs, particularly focusing on individuals with a history of trauma (e.g. sexual abuse, domestic violence, community violence, physical abuse).
