About The Position

CEC Health Care, a leading not-for-profit organization in Nassau County, is seeking a dedicated and compassionate Social Worker or Mental Health Counselor to join our Assertive Community Treatment (ACT) Team. Our mission is to help adults diagnosed with serious mental illness achieve independence, self-determination, and wellness. As a member of our dynamic, multidisciplinary ACT team, you will play a vital role in delivering person-centered, community-based mental health services that empower clients to Aspire Higher.

Requirements

  • Master’s degree in Social Work or Mental Health Counseling, required.
  • Current NYS License or Limited Permit in LMSW, LCSW, or LMHC.
  • Valid NYS driver’s license and ability to travel throughout Nassau County.

Nice To Haves

  • At least one year of experience working in behavioral health with a focus on health and wellness initiatives, preferred.

Responsibilities

  • Provide mobile, community-based mental health counseling and support throughout Nassau County.
  • Provide individual and group counseling to youth and young adults, supporting emotional and behavioral wellness.
  • Conduct comprehensive needs assessments and develop individualized recovery plans.
  • Collaborate with clients to build skills that support recovery and and long-term goal achievement.
  • Deliver family psychoeducation (individually and in groups) to enhance communication and strengthen support systems.
  • Integrate natural supports and community resources into treatment with client consent.
  • Work closely with a multidisciplinary team to coordinate and integrate services.
  • Monitor the quality and effectiveness of support services and adjust interventions as needed.
  • Participate in on-call rotation to ensure 24/7 crisis response coverage.
