About The Position

Under supervision, performs a variety of case management services for the Perinatal Health Partners program. This role involves promoting population health through illness, injury, and disability initiatives, delivering home visits for perinatal patients and infants, and providing care coordination and referrals. The Social Worker will also help staff contact patients, process initial referrals, coordinate services with healthcare providers, agencies, and community organizations, and build/maintain community partnerships. Responsibilities include representing the program at outreach events, maintaining accurate patient records, service documentation, and treatment outcomes, and assisting with inventory management and distributions. A key duty is performing clinical assessments of behavioral health needs using the Edinburgh Postnatal Depression Scale and referring clients or families to community resources for recovery and access to services like transportation, financial assistance, legal aid, housing, job placement, or education. The role also requires becoming a certified Child Passenger Safety Technician to educate families receiving car seats. This position supports multiple counties: Bleckley, Dodge, Johnson, Laurens, Montgomery, Pulaski, Telfair, Treutlen, Wheeler, and Wilcox. Mileage is reimbursed.

Requirements

  • Bachelor’s degree in a related field from an accredited college or university
  • Two (2) years of experience in social service delivery.

Responsibilities
