This role provides psychosocial assessment, counseling, crisis intervention, and information/referral services on a consultation basis, working collaboratively with the management team to ensure efficient resource utilization for positive patient outcomes.
Requirements
- Master’s Degree in Social Work
- 1 year experience working as a social worker or applicable mental health experience.
- Ability to work effectively with people from a broad range of backgrounds.
- Ability to adhere to the National Association of Social Workers Code of Ethics.
- Knowledge of patient rights and laws relative to those rights, such as HIPAA.
- Knowledge of psychological principles and theories.
- Must possess a high degree of emotional intelligence, and empathy towards others.
- Skill in using logic and reasoning to identify the strengths and weaknesses of alternative solutions, conclusions, or approaches to problems.

Responsibilities
- Employs counseling skills to help patients/families understand, accept, and follow recommendations for care, and to restore patients to optimum social and health adjustment.
- Stays up-to-date on current trends and researches the availability of community agencies and community resources for social, emotional, and financial assistance.
- Intervenes in crisis situations, and effectively applies crisis intervention theory techniques.
- Identifies and records clinical, psychosocial, and financial barriers to a smooth transition across the healthcare network.
- Participates in a monthly or yearly audit or review to ensure adherence to laws and other federal guidelines.
- Works collaboratively with patients and their social support systems to establish and implement a plan.
- Interviews patients to complete the initial psychosocial evaluation.
- Reports suspected cases of child and adult abuse/neglect/exploitation.
