The MCC Social Worker - LMSW provides intensive social work support of patients in the outpatient infusion center. Patient populations served range in age from 17 years (the minimum age of patients at infusion clinic) through patients in late stage of the life span. This includes a variety of support for clients with serious or severe physical illness in the oncology setting, experiencing end of life issues, caregiver burnout or complex psychological/social barriers that interfere with obtaining maximum benefits from qualifying programs. The LMSW provides social casework with individuals, families, groups, and communities to locate resources, negotiate, and advocate on behalf of clients or client groups. The LMSW evaluates and assesses difficulties in psychosocial functioning, develops a plan to alleviate those difficulties and either carry out the plan or refer clients to other qualified resources for assistance. The LMSW may provide preventive and treatment services pertaining to individuals, families, groups, communities and organizations. The LMSW engages the client system, develops relationships and employs a series of interventions that modify or eliminate targeted behaviors and promotes positive growth and development. The LMSW can explain and interpret the results of social evaluations in the problem-solving process. Services are performed in person or telephonically and interventions are recorded in the population care management application. This position serves on the cancer program steering committee, coordinating ongoing assessment of programmatic goals and metrics. This position trains clinical personnel who contribute to the distress screening process.
