About The Position

Responsible for facilitating the care of designated patients and their families during a patient's hospital admission or outpatient visit. Coordinates discharge plan from pre-admission through discharge follow up. Provides psychosocial assessment and updates that address the patient's evolving needs. Establishes a plan of care that provides maximum benefit to patient and their families. Other duties as assigned.

Requirements

• Masters Degree in Social Work with casework in medical social work.
• Interpersonal skills necessary to obtain information from patients, to provide counseling to patients/families, and interact effectively with patients, families and hospital personnel.
• Analytical skills necessary to assess patients needs, develop associated discharge planning and develop solutions to problems concerning patients mental and physical well being.
• Current State of Wisconsin license as social worker required.

Nice To Haves

• Minimum of two years experience in an acute care setting preferred.
