About The Position

Responsible for coordinating the care of patients and their families or significant others within a level 1 Trauma Emergency room (ED) setting. Collaborates with local law enforcement, 414LIFE navigators, housing navigator, Milwaukee County Health Care Partnership program, 2-1-1, and other local community partners as appropriate. Monitors admission status, medical appropriateness, supports grieving families, and coordinates discharge Planning/end of life coordination. Has the ability to multitask multiple consults in a fast paced, high stress trauma ED environment 7 days a week.

Requirements

• A minimum of 2 years of experience in an acute care setting is required.
• Master's Degree in Social Work.
• Interpersonal skills that are necessary to obtain information from patients, to provide counseling to patients/families, and interact effectively with patients, families, and hospital personnel.
• Analytical skills associated with the patient's discharge and to be able to develop solutions to problems concerning patient's mental and physical wellbeing.
• Current State of Wisconsin licensure as a Social Worker.
