About The Position

The primary focus of the Social Worker position is pediatric diabetes ambulatory care with an emphasis on adolescent issues. Through in-depth psychosocial assessments, the Social Worker will identify child and family strengths, stressors, problem-solving abilities, dynamics, interactions, attitudes, feelings and cultural issues in relation to youth living with diabetes. The Social Worker will work collaboratively with the team to perform interdisciplinary assessments aimed at developing and implementing a plan of intervention, which addresses education, coping and the managing the burden of treatment. This will be achieved through a variety of modalities including individual and family therapy, group work, consultation, case management, crisis intervention, patient and family education, community outreach and advocacy. The Social Worker will identify and make referrals to appropriate supports and resources and assist families in accessing financial assistance programs when necessary.

Requirements

  • Master’s Degree in Clinical Social Work from an accredited School of Social Work
  • Current certificate of Registration with the Ontario College of Social Workers and Social Service Workers
  • Recent (3-5 years) experience with children and families in acute pediatric setting
  • Previous clinical experience in individual and family therapy
  • Familiarity with the challenges facing children and families managing a chronic illness
  • Knowledge of community resources and referral process
  • Familiarity with the Mental Health Act, Consent to Treatment Act, Child and Family Services Act
  • Demonstrated expertise in assessment, counseling, crisis intervention and advocacy skills in working with adolescents and families coping with mental health issues
  • Excellent communication and teamwork skills
  • Demonstrated ability to work in a fast-paced, changing environment
  • Demonstrated commitment to ongoing learning and the ability to teach
  • Proven ability to organize, prioritize and problem-solve effectively

Nice To Haves

  • Certified Diabetes Educator (CDE) qualifications are an asset, or working toward completion of CDE qualifications

Responsibilities
