About The Position

Barnes-Jewish West County Hospital is looking to hire a Social Worker (MSW). This role involves assisting patients and their families in various settings to adapt and plan an interdisciplinary approach to treating the patient-family system. The Care Coordination department, which includes Case Management, Social Services, and Utilization Review, is responsible for discharge planning, ensuring proper patient billing, and maintaining compliance with payor requirements and regulations. Employees working at an Illinois facility must have an Illinois state social work license or be able to obtain one within 90 days of employment.

Requirements

• Master's Degree of Social Work (MSW) – required

Nice To Haves

• Ability to set up oxygen, home health and placement out of the ED setting

Responsibilities

• Provides direct clinical services to patients and families.
• Plans and implements social work services on assigned clinical service or unit in collaboration with the healthcare team.
• Collaborates with community human service agencies to meet the needs of patients, families, and the hospital.
