About The Position
Andwell Health Partners is seeking a compassionate and dedicated Medical Social Worker to join our healthcare team. In this role, you will provide psychosocial support, counseling, care coordination, and community resource referrals to patients and families navigating complex medical conditions.
Requirements
- Degree in Social Work (or related field) from an accredited program.
- Current social work licensure or eligibility for licensure, as required.
- LSX Minimum.
- Strong communication, assessment, and care coordination skills.
Nice To Haves
- Experience in healthcare, home health, hospice, or community-based services preferred.
Responsibilities
- Assess patients’ social, emotional, and environmental needs.
- Develop and implement individualized care plans.
- Provide counseling, crisis intervention, and patient advocacy.
- Collaborate with physicians, nurses, and interdisciplinary team members.
- Connect patients and families with community resources and support services.
- Maintain accurate documentation in compliance with organizational and regulatory guidelines.
