About The Position

This is a temporary, part-time position for a Social Worker with a Master's degree, to work in the Regional Bariatric Program. The role involves providing services to patients with chronic diseases, with a preference for bariatric experience. The position requires current registration with the Ontario College of Social Workers and Social Service Workers and involves working with an interdisciplinary team. The role also requires a valid driver's license and the ability to provide own transportation, as well as a Criminal Reference Check.

Requirements

  • Master’s Degree in Social Work required.
  • Bachelor’s Degree in Social Work required.
  • Current registration in good standing with the Ontario College of Social Workers and Social Service Workers.
  • Ministry of Labour “Worker Health and Safety Awareness in 4 Steps” training certificate is required.
  • Recent and relevant experience (minimum 3 years) in the field of Chronic Disease.
  • Competencies in assessment, treatment and counseling of patients with chronic disease.
  • Proven knowledge and understanding of best practices as it relates to obesity and associated co-morbidities.
  • Proven in-depth knowledge of psychological factors affecting patients with obesity and in-depth understanding of the psychosocial, physical and functional problems endemic to obesity.
  • Knowledge of DSM-IV and psychosocial issues affecting seriously mentally ill clients and those with addictions.
  • Proven knowledge of the Consent to Treatment Act, Substitute Decisions Act, Consent and Capacity Board and Powers of Attorney.
  • Skills in a range of individual, family and group therapies as well as experiential therapies.
  • Proven ability to foster and improve relationships between professionals, programs and agencies.
  • Excellent written and oral communication skills.
  • Computer skills and experience with information technology (video conferencing) for consultation, assessment and training purposes.
  • Proven leadership, problem solving and organizational skills.
  • Personal suitability for working independently with an interdisciplinary team.
  • Proven organizational skills and ability to prioritize workload is essential.
  • Demonstrated positive work record and good attendance record.
  • Valid driver’s license and must be able to provide own transportation.
  • Criminal Reference Check is required.

Nice To Haves

  • Preference for bariatric experience.
  • Experience in community-based, out-patient program or regional program preferred.
  • Experience in group presentations and teaching, and community mental health service delivery preferred.
