About The Position
Atlantic Health System is seeking a Per diem Social Worker within the Atlantic Homecare & Hospice division. This role involves providing home visits to home care patients, offering disease counseling, conducting end-of-life discussions and goals of care, making community resource referrals, and assisting with applications to community resources.
Requirements
- Master's in Social Work from an accredited school of social work required
- Licensed in the state of NJ (LSW)
- Use of personal vehicle is required for home visits.
- Valid Driver’s License with a clean record required.
- Excellent customer service and communication skills.
Nice To Haves
- State of NJ LCSW preferred
- 1-2 years’ experience in social work preferred
- Expertise in community resources preferred
- Bilingual a plus
Responsibilities
- Performs comprehensive psychosocial assessments, identifying issues impacting patient and family safety and assessing support systems for patient/family goals.
- Collaborates with other disciplines in the development of the plan of care, providing input for goals, participating in the interdisciplinary team, and collaborating with external resources.
- Collaborates with other disciplines in the implementation of the plan of care, counseling patients and families on coping strategies, participating in family conferences, and providing referrals to community services.
- Evaluates the effectiveness of social work services.
- Maintains high standards of professional practice, including completing documentation and staying current on community resources and eligibility requirements.
- Participates in the agency’s Performance Improvement program, including utilization review activities.
- Assists in the development of agency policies and procedures, including orienting new employees and participating in revising job functions.
- Assists with growing agency business through community education and outreach, and accepts other assignments as requested.
- Other Tasks as assigned.
