About The Position

Apple Rehab West Haven is a respected 90-bed facility deeply ingrained within its community, renowned for its commitment to exceptional care. Our senior management is based in our local Avon, CT office, ensuring superior care from a company deeply rooted in your community. This role is for an LPN Charge Nurse on a per diem basis, with competitive rates offered based on experience. Available shifts are half shifts from 8am-12pm or 4pm-8pm.

Requirements

Valid state nursing license in good standing (or in the process of becoming a registered nurse).
In-depth knowledge of nursing practices, medical procedures, and long-term care regulations.
Strong attention to detail, excellent organizational skills, and the ability to manage multiple tasks effectively.
Collaborative team player.
Per diem staff are expected to fulfill (2) weekend shifts monthly to meet requirements.

Nice To Haves

Previous experience in long-term care preferred.

Responsibilities

Planning, executing, and overseeing nursing services for assigned residents.
Adhering to quality care standards, facility policies, legal requirements, medical practices, and physician instructions.
Efficiently planning and providing professional care while aligning with the needs of residents and the changing medical landscape.
Maintaining professionalism in interactions with residents, physicians, families, peers, and support staff.
Documenting crucial information related to resident care accurately and comprehensively.
Administering prescribed medications and overseeing accurate record-keeping in line with facility policies.
