About The Position
The Community Hospice has an opportunity for a social worker to join our hospice team caring for patients in homes and nursing facilities as part of our Interdisciplinary Team. As a member of the hospice team, the primary focus is to respond to the emotional and psychological needs of patients, families and staff. Position is Saturday and Sunday 8am-8pm.
Requirements
- NYS SW Licensure
- Valid NYS Drivers license
- Minimum 2-3 years experience ideally caring for seniors
Responsibilities
- Diagnose and treat psychological, behavioral and emotional disorders
- Provide specialized skills and mental health therapy for clients.
- Identify barriers which impact optimal patient functioning
- Ensure patients receive appropriate resources in most appropriate setting
