About The Position

Vanderbilt Health is the region's largest comprehensive academic health system, comprising eight hospitals and over 180 clinics. This role involves providing psychosocial assessments and brief targeted interventions for social, emotional, and environmental issues that affect a client/family's ability to benefit from care. The position actively collaborates with the healthcare team to achieve patient care management and system improvement goals, and represents the Social Work department on institutional and community committees. The role contributes to the plan of care by understanding population-specific needs related to identifying patient psychosocial needs.

Requirements

LIC-Licensed Master Social Worker - Tennessee
Master's degree in Social Work
Less than 1 year of relevant work experience
Possesses fundamental proficiency in risk management, crisis intervention, patient assessment, patient evaluation, and clinical counseling, with the ability to apply these skills in practical applications of moderate difficulty.

Responsibilities

Maintains professional documentation that meets department and VUMC standards for compliance.
Provides brief targeted interventions for identified clients.
Completes psychosocial assessments and assists the team in developing the plan of care.
Provides education and/or consultation to clients, families, the treatment team, and the community.
Contributes to the organization by actively participating in process improvement initiatives and shared decision-making.
Performs additional duties as assigned.
