Hospice Social Worker - LMSW
About The Position
Spirit Hospice is seeking an additional licensed Social Worker (LMSW) to join their team. This role is considered an equal pillar to nurses and chaplains, requiring someone who can support patients and families through difficult life transitions with grace, strength, and compassion. The Social Worker will help families navigate grief, fear, and administrative tasks, while advocating for dignity and love even in times of loss. Collaboration with an interdisciplinary team (IDT) including nurses, chaplains, aides, doctors, and volunteers is essential for developing and executing patient-specific care plans. The role involves offering resources, counseling, and a supportive presence.
Requirements
- A current Social Work License in the state of Georgia.
- Hospice, medical, or mental health experience. (and if not hospice yet, a calling to it)
- A steady heart that can hold both tears and laughter in the same breath.
- Strong communication, organizational, and advocacy skills.
Responsibilities
- Walk alongside patients and families through some of life’s hardest but most meaningful moments.
- Help families navigate the messy parts.. grief, fear, and paperwork.
- Lead with compassion, advocate fiercely, and remind people that even in loss there’s still love and dignity.
- Collaborate closely with nurses, chaplains, aides, doctors and volunteers in our interdisciplinary team (IDT), ensuring care is heart-centered.
- Develop and execute patient-specific care plans
- Offer resources, counseling, and that grounding presence people remember long after the goodbye.
