About The Position
The role of the Social Worker reports to the Center Manager. This position is responsible for assisting patients and their families with personal and environmental difficulties affecting their illnesses and subsequent medical care.
Requirements
- Must be a graduate of an accredited Masters of Social Work program
- LMSW, required.
- Previous experience as a social worker in an acute care setting, required.
Responsibilities
- Maintain effective working relationships with internal and external clients
- Identify and analyze social and emotional factors underlying patient illness
- Collaborate with interdisciplinary team to perform interventions, as needed
- Assist patient and family with procurement of resources and applicable educational material
- Assist with discharge planning process
