Requirements

  • Licensed Master Social Worker through the Louisiana State Board of Social Work Examiners

Responsibilities

  • Completes psychosocial assessment, develops plans, and carries out interventions for patients identified with psychosocial risk factors.
  • Prioritizes timely response to referrals based on urgency of need.
  • Conducts assessments of patient's psychosocial needs through interviewing, interdisciplinary team conferences, and medical record reviews.
  • Evaluates coping skills, cognitive functioning, support systems, and resources that could affect responses to illness, treatment, and discharge.
  • Identifies barriers and plans interventions to overcome them.
  • Communicates findings and plans to the interdisciplinary team and documents assessments, plans, and interventions.
  • Provides short-term supportive counseling for individuals experiencing temporary or situational problems.
  • Performs assessments for suspected elder, child, sexual, or domestic abuse or neglect.
  • Complies with required reporting according to state law and hospital policy.
  • Refers patients/families to appropriate community agencies for further intervention or counseling.
  • Facilitates interactions between staff and DCFS/EPS or other agencies.
  • Acts as an active team member in the discharge planning process and ensures patients are referred to appropriate social and financial resources post-discharge.
  • Identifies patients with complex social and medical issues through case finding and referral.
  • Reviews caseload with Manager to share findings, needs, barriers, and progress to discharge.
  • Evaluates financial assistance needs and eligibility and directs patients/families to appropriate community agencies.
  • Maintains a working knowledge of payor reimbursement requirements for post-hospital services.
  • Maintains a working knowledge of available community resources by establishing relationships with liaisons and admissions staff at agencies and facilities.
  • Demonstrates the ability to address legal/ethical issues regarding healthcare as it relates to medical social work.
  • Demonstrates knowledge of Advance Directives and patient rights.
  • Counsels/educates patients/families regarding patient rights, decision-making, and formulating Advance Directives.
  • Facilitates family meetings to aid in decision-making when there are disagreements or lack of clarity around goals of care and plan of care.
  • Links patients and families to available resources in the hospital and community for ongoing support, such as Hospice and Palliative Care.
