About The Position

This is a part-time Social Worker position requiring 20 hours per week, with work scheduled for Monday, every other Wednesday, and Friday. The role involves collaborating with an interdisciplinary team to assist patients and families facing complex psychosocial needs. Interventions include, but are not limited to, alternate site coordination, discharge planning, addressing abuse/neglect, adoption, substance use disorders, end-of-life decisions, providing referral resources, bereavement support, adjustment counseling, facilitating guardianships, and managing behavioral health concerns (including psychiatric transfers). The Social Worker will conduct psychosocial assessments, communicate with external sites and agencies, document assessments and plans in the electronic medical record, provide consultation to the healthcare team, maintain knowledge of community resources, and participate in relevant committees. The role also requires staying current with federal and state regulations and providing short-term therapeutic support as appropriate.

Requirements

  • Master's Degree Social Work Required.
  • Master Social Worker (MSW-Master) - State of Michigan Upon Hire Required.

Nice To Haves

  • 2 years of relevant experience health care related experience post obtaining Preferred.
  • Care management experience Preferred.
  • Experience in individual, family assessment, crisis intervention, grief and loss counseling, and discharge planning preferred.

Responsibilities

  • Work collaboratively with members of an interdisciplinary team to assist patients and families with complex psychosocial needs.
  • Conduct psychosocial assessments with patient and/or family to assist with identified psychosocial needs or who may be identified to require an alternate site of care post discharge and execute a plan.
  • Communicate with alternate sites of care and/or community agencies in an effective and timely manner, to best address the patient’s needs.
  • Complete and document the assessment and plan in the electronic medical record, accurately reflecting the patient’s current condition, situational factors, transition of care needs and psychosocial imperatives.
  • Provide consultation and resources to members of the healthcare team.
  • Maintain relevant and current knowledge of community resources.
  • Participate or assist in department, regional, or statewide stakeholder committees, projects etc., as assigned/requested.
  • Develop and maintain current knowledge of federal and state regulations as they pertain to role.
  • Provide short term therapeutic support as appropriate for setting and location.
