About The Position

Vanderbilt University Medical Center (VUMC) is seeking a Social Worker LMSW to join their TMO/UM Team D. This role involves providing psychosocial assessments and brief targeted interventions for social, emotional, and environmental issues that impact a client/family's ability to benefit from care. The Social Worker will collaborate with the team to meet patient care management and system improvement goals, represent the Social Work department on institutional and community committees, and provide leadership for department committees/workgroups. The position requires contributing to the plan of care with knowledge of population-specific needs related to patient psychosocial needs. The role involves working two 10-hour weekend shifts and two 10-hour float shifts during the week, with incentive pay for weekend shifts. Responsibilities

Maintains professional documentation which meets department and VUMC standards as it relates to compliance.
Provides brief targeted interventions for identified clients.
Completes psychosocial assessment and assists team with the development of the plan of care.
Provides education and or consultation to clients and families, treatment team and community.
Contributes to the organization by actively participating in process improvement initiatives and shared decision making.
