About The Position
Excellent opportunity for a FT Licensed Master Social Worker (LMSW) to work in a hospital setting! This role provides continuum of care for patients from admission to discharge. The Social Worker assesses the social, emotional, and financial needs of the patient and assists in developing discharge plans with patients and their families. This includes referrals to community resources and/or placement in an appropriate facility, following discharge.
Requirements
- Masters degree from a recognized school of Social Work accredited by the Council on Social Work Education
- Minimum one year of job related experience preferred in an acute care setting
- Knowledge of Medicare/Medicaid regulations
- Knowledge of community resources
- Current LMSW South Carolina License
- BLS
