About The Position

Cleveland Clinic Weston Hospital is seeking a Social Worker (LCSW) to coordinate social work services for patients and families, including psychosocial assessment, counseling, resource information, and referrals. The role is not limited to oncology patients. The social worker supports patients and loved ones in developing coping skills throughout illness, hospitalization, treatment, and continuing care, and serves as a patient safety ombudsman and advocate for transparency and disclosure in the event of medical errors. The caregiver works days from 8:00 a.m. to 5:00 p.m. The role involves caring for chronically ill patients, completing and monitoring care plans, consulting with medical disciplines, coordinating referrals and financial resources, providing crisis intervention and bereavement support, and participating in multidisciplinary care planning. The position is based at Florida Weston Hospital in the Hematology Oncology department.

Requirements

Master’s degree in Social Work.
Licensed Clinical Social Work (LCSW).
Effective verbal, written, and computer skills.

Nice To Haves

Certification as Breast Patient Navigator in Breast Imaging and Cancer Care (CBPN-IC).
Three to five years of clinical social work experience.
Two years of hospital Care Management experience.
Knowledge of InterQual guidelines.
Hospital or hospice experience.
Strong behavioral health background.

Responsibilities

Coordinate social work services, including psychosocial assessment, counseling, resource information, and referrals for patients and families.
Provide care for chronically ill patients using knowledge of human development and ethnic, cultural, and spiritual influences.
Apply knowledge of oncology diseases and treatments, including patients’ understanding, reactions, and expectations of the disease process.
Complete timely psychosocial assessments and develop individualized short- and long-term care plans.
Assess and monitor treatment goals as patients adjust to chronic illness and changes in functional status.
Consult with medical disciplines and staff about biopsychosocial, environmental, ethnic, and cultural factors affecting adjustment to chronic illness and treatment.
Educate patients and families and develop clinical care plans with multidisciplinary teams based on mutually agreed goals.
Partner with outpatient teams and insurance and billing specialists to implement policies related to social work and outpatient issues.
Assist in securing financial coverage for treatments and medications consistent with quality patient care.
Analyze and facilitate referrals to community resources and governmental health and welfare programs.
Provide crisis intervention, use high-risk screening criteria for case finding, and conduct outreach that may involve mental health and chemical or ETOH dependency resources.
Initiate multidisciplinary team conferences for complex cases and advocate for patients’ dignity, confidentiality, rights, and access to care.
Provide bereavement support, therapeutic counseling, and referrals for palliative disease management, hospice, and end-of-life issues.
Serve as a patient safety ombudsman and advocate for transparency and disclosure in the event of medical errors.
