Job Summary Responsible for conducting psychosocial assessments, providing individual, group, and family counseling, and coordinating care for patients and families across diverse clinical settings. Delivers education, support, and therapeutic interventions to address emotional, behavioral, and practical needs throughout the continuum of care. Identifies barriers to treatment, facilitates referrals to internal and external resources, and collaborates with multidisciplinary teams to optimize patient outcomes. Documents patient interactions, participates in risk assessments, and supports patient safety planning. Engages in program development, quality improvement, and research initiatives to enhance social work services and patient well-being.

Requirements

  • Master’s degree in social work
  • 3-5 years of experience in social work, or a relevant area
  • Licensed Certified Social Worker (LCSW) license
  • All experience must be paid and in the same related field.
  • Part-time and PRN experience will be prorated based on hours worked per week.
  • Volunteer work and internships for academic credit are not counted.

Responsibilities

  • Completes individualized psychosocial assessments to identify emotional, social, environmental, and financial factors impacting patient care.
  • Provides direct patient care, including crisis intervention, counseling, education, advocacy, referral management, and mediation across age groups and populations.
  • Assists with screening, identification, and intervention for suspected abuse, neglect, intimate partner violence, exploitation, and sexual assault, and coordinates with appropriate agencies.
  • Collaborates with interdisciplinary teams, courts, and community partners to develop, implement, and revise care plans, ensuring representation of the social work perspective.
  • Monitors, evaluates, and documents patient progress toward measurable goals, updating care plans as needs change and maintaining compliance with confidentiality and documentation standards.
  • Connects patients and families with community, state, and national resources to address gaps in healthcare, support care transitions, and enhance access to services.
  • Provides education, training, and support to patients, families, staff, and students, and participates in professional development and continuing education activities.
  • Develops and implements programs to address psychosocial, educational, financial, and healthcare needs.
  • Participates in quality improvement, research, and data analysis initiatives to enhance social work practice and patient outcomes.
  • Performs other duties as assigned.
