This Social Worker III position performs work to support the mission, goals and values of HHSC and applies critical thinking skills and sound decision making. Work performed supports rehabilitation outcomes for individuals with traumatic brain injury (TBI) and/or spinal cord injury (SCI) utilizing a wraparound approach, person-centered planning and trauma informed care. The role involves collecting and analyzing information to determine eligibility, working collaboratively with individuals to develop a person-centered rehabilitation plan of supports, providing case management, resource facilitation, continuous reassessment of needs, and discharge planning. The Social Worker III will counsel individuals and their families, oversee the treatment process with contractors, develop and maintain relations with community referral sources and stakeholders, and provide education, coaching, and mentoring. This role applies best practices and uses available comparable benefits and services to ensure effective use of public funds. The position serves as the subject matter expert for TBI and SCI, works using the HHS electronic case management system, and operates independently with minimal supervision.
