The University of Iowa Healthcare department of Care Coordination is seeking a Social Worker III to provide direct social work services including assessment, treatment, interventions, and recommendations to facilitate movement through the continuum of care to patients and their families. This role will also participate in and provide the social work perspective to interdisciplinary activities, as well as provide social work treatment, individually or in groups, to patients and clients and their families with complex emotional or social problems. Additionally, this position assumes basic consultation, supervisory, teaching, research, program development, and/or administrative responsibilities. The department intends to hire either a Social Worker II or a Social Worker III. Duties include developing and implementing a plan of care responsive to patient/family concerns, health care status, resources, age-specific problems, and clinical impressions. Identifying patients requiring social work intervention, performing social work assessments, developing clinical impressions, and providing a range of social work services. This role also involves providing information, consultation, and professional support to interdisciplinary team members, acting as a resource, and advocating for the social work role. Developing and facilitating groups for patients and/or significant others, and contributing to the strengthening of the social work profession are also key aspects of this position.
