Performs difficult professional work in the support and assistance of agency clients with personal, social, and economic needs. This role involves assessing needs, developing service plans, managing cases, maintaining records, and preparing reports. The Social Worker will provide assistance in areas such as adult protective services, guardianships, representative payee services, and placement services. Responsibilities include making home visits to assess risk to adults and family services needs, developing intervention plans, performing casework, and arranging for transportation and referrals to ancillary services. The position also involves arranging placements in day care centers, nursing, and/or rest homes, preparing for and attending court hearings, staff meetings, and conferences. Additionally, the role may involve monitoring adult care and adult day care facilities for compliance, investigating licensure complaints, and serving as a technical consultant to facilities. The position requires being on-call on a rotating basis.
