About The Position

Centinela Hospital Medical Center is seeking a Social Worker II to support patients and their significant others across Pediatrics, Adolescents, Adults, and Older Adults. The role provides emotional support, crisis intervention, counseling, information and referrals, and referrals for financial assistance and supportive services for needs related to hospitalization, illness, disability, or trauma. The Social Worker II provides case and milieu management, completes psychosocial assessments, assists with the development and re-evaluation of treatment plans, participates as a member of a multidisciplinary team, and coordinates discharge planning. The role also requires completing necessary documentation and maintaining a clinical record for each client throughout their length of stay.

Requirements

Master’s degree in Social Work, Marriage and Family Therapy, or Professional Clinical Counseling.
In states with a licensure requirement, obtain licensure upon hire and maintain it in accordance with state regulations.
At least one year of inpatient or outpatient experience with an emphasis in mental health care.
Experience working with a diverse population and a wide range of problems, such as death and dying, maternal and childcare, substance abuse, disability, and chronic illness.
