About The Position
Provides assessment, referral, education, service planning, and care coordination services to CC clients with a focus on increasing self-management and adherence to medical and behavioral health treatment plans. Works as a part of an interdisciplinary team with CrescentCare and/or community providers. Maintains a close working relationship with direct supervisor, participates in active supervision and remains receptive to feedback on professional development to support high quality service provision
