About The Position
ChristianaCare is seeking an energetic, motivated, and compassionate Social Worker with an MSW to join their team. This role is crucial in resolving psycho-social barriers and supporting discharge needs within a collaborative Care Management Triad Team Model. The position is located at the Newark campus and operates on a 3p-11:30p shift. A $15,000 sign-on bonus is available for eligible candidates with a two-year commitment.
Requirements
- Master’s degree in Social Work (MSW) or Master of Social Services degree (MSS) from an accredited graduate school required prior to hire date.
- Licensed CSW or MSW as required by the state of Delaware prior to hire date.
Nice To Haves
- Acute Care Social Work experience preferred.
- CCM preferred within 18 months of eligibility.
Responsibilities
- Resolve psycho-social barriers and supports discharge needs.
- Manage patient care and drive patient progression.
- Establish a discharge plan.
- Review patient status for appropriateness and anticipated payer coverage.
