About The Position

Vi at Bentley Village is seeking a compassionate, resident-focused Social Worker to join our Skilled Nursing Facility team. In this role, you will advocate for residents, support families, and help ensure the highest quality of care and quality of life for those we serve. The Social Worker is responsible for assessing residents' psychosocial needs, coordinating services, facilitating care planning, and providing emotional support to residents and families throughout their stay. This position works collaboratively with the interdisciplinary team to promote resident well-being, successful transitions of care, and compliance with regulatory requirements.

Requirements

2 years related experience
At least 1 year of experience in a health care setting (hospital, clinic, rehabilitation center, etc.)
Bachelor's Degree in Social Work, Psychology, Gerontology or Sociology from an accredited institution
Excellent problem solving skills, using a customer-focused approach to problem solving and goal setting
Excellent communication and listening skills
Enthusiastic, resourceful, organized
Possesses strong personal awareness of nonverbal communication
Computer skills including proficiency in MS Office
Compliance with Florida’s Care Provider Background Screening process
