About The Position

Paradise Valley Hospital is seeking a Social Worker II to support patients and their significant others across pediatric, adolescent, adult, and older adult populations. The role provides emotional support, crisis intervention, counseling, information and referrals, and referrals for financial assistance and supportive services for challenges related to hospitalization, illness, disability, or trauma. The Social Worker II provides case and milieu management, completes psychosocial assessments, assists with the development and re-evaluation of treatment plans, works as part of a multidisciplinary team, and coordinates discharge planning. The role also requires completing necessary documentation and maintaining a clinical record for each client throughout their stay.

Requirements

Master’s degree in Social Work, Marriage and Family Therapy, or Professional Clinical Counseling.
Where state licensure is required, obtain licensure upon hire and maintain it in accordance with state regulations.
At least one year of inpatient or outpatient experience with an emphasis in mental health care.
Experience working with a diverse population and a wide range of problems, such as death and dying, maternal and childcare, substance abuse, disability, and chronic illness.
