About The Position

The Social Worker II MSW provides varied and ongoing social work services to hospice patients and their families. Services may include psychosocial evaluation, ongoing psychosocial counseling, direct casework, bereavement services, and community outreach and referrals. The role collaborates with physicians, staff, families, community agencies, and hospice leadership, and actively participates in quality monitoring and performance improvement activities.

Requirements

• Master's degree in Social Work.
• Counseling or prior medical social work training.
• Licensed in Iowa or Illinois upon hire; obtain a license in the second state within 12 months.
• Valid driver's license and auto insurance meeting Genesis minimum limits.
• More than 2 years of experience.
• Strong interpersonal skills to handle difficult or stressful communications, sensitive and complex topics, complicated problem situations, and interactions requiring agreement from others.

Responsibilities

• Provide psychosocial evaluations for hospice patients and their families.
• Provide ongoing psychosocial counseling and direct casework services.
• Provide bereavement services.
• Conduct community outreach and referrals in collaboration with physicians, staff, families, community agencies, and hospice leadership.
• Participate in quality monitoring and performance improvement activities.
