About The Position

Piedmont Rockdale is a 161-bed, acute care, not-for-profit community hospital in Conyers that has provided compassionate, patient-centered care to Rockdale County and surrounding communities since 1954. We offer 24-hour emergency care, plus most major medical, surgical and diagnostic services.

Requirements

• Master’s Degree in Social Work from an accredited social work program
• 1 year of experience in an acute or post-acute setting

Nice To Haves

• LMSW - Licensed Medical Social Worker - State Licensure
• Accredited Case Manager (ACM)
• CCM - Certified Case Manager

Responsibilities

• Promote patient and family wellness, improved care outcomes, access to appropriate hospital and community resources.
• Manage, support, and develop comprehensive transitional care plans for patients with complex and psychosocial needs in Inpatient and Emergency Department (ED) environments.
• Promote quality of care and effective transitions planning through collaboration with all service team members, physicians, patients, external agencies and families.
