About The Position

Responsible for providing therapy, case management services and linkages to community agencies to meet patients' social and emotional needs. Identifies and assesses client patient cases and anticipates their needs related to illness, hospitalization, finances, education and home or community services. Serves as a liaison between health care facilities, community resources and government agencies. Provides individual and group therapy to children and adolescent patients in an outpatient community psychiatry treatment clinic located within an academic medical center. Interviews patient, family and any other providers to assess the patient's level of functioning, strengths and deficits, and emotional status. Develops a comprehensive picture of the patient’s treatment needs. Serves as a member of the treatment team in the diagnostic evaluation of patients and the identification of treatment objectives. Provides evidenced-based treatment for patients at CDT. Provides group, Family-Based Treatment and individual treatment. Provides therapeutic support. Program therapists develop treatment plans, monitor and document progress and facilitate discharge planning to meet compliance and billing requirements and successfully return the patient to community resources. Program therapists work within a multidisciplinary team of therapists, medical providers and families to ensure comprehensive treatment. Participate in weekly clinical team meetings with multi-disciplinary team of medical providers and therapists.

Requirements

  • Master's degree in a social work or social welfare program accredited by the Council on Social Work Education.
  • MSW licensure by the South Carolina Board of Social Work Examiners.

Nice To Haves

  • Certification in case management preferred.
