About The Position

This Social Worker II position is responsible for providing intake services for a broad range of social work programs in the agency. The worker assesses problems clients identify, counsels with them regarding possible solutions or refers clients to resources available in the community. The worker will take and process applications for Energy and Emergency Assistance Programs. They will refer clients to other agencies/programs that can provide possible assistance. This worker will serve in the afterhours on-call rotation. During office hours this worker will serve as back-up for Adult Protective Services to take reports and initiate evaluations of protective services reports. This position provides representative payee services and maintains a SA In-Home caseload. Position may be assigned other related duties and activities as deemed appropriate by the supervisor.

Requirements

Considerable knowledge of social work principles, techniques, and practices and their application to specific casework, group work, and community problems.
Knowledge of psychosocial, socioeconomic, and behavioral problems and their treatment.
Knowledge of governmental and private organizations and community resources.
Knowledge of laws, regulations, and policies which govern the program.
Knowledge of the application of information technology to the work.
Working knowledge of standard office procedures, practices, and equipment.
Skill in teamwork, collaborative conflict resolution and customer service excellence.
Skill to use a variety of office machines, computers, applicable software and typing.
Skill in establishing rapport with a client and applying techniques of assessing psychosocial, behavioral, and psychological aspects of client's problem.
Ability to establish and maintain effective working relationships with administrative supervisors, members of case load and their families, and with care providers and various community organizations.
Ability to express ideas clearly and concisely.
Ability to plan and execute work.
Ability to establish and follow work procedures, prioritize work, and prepare and maintain accurate records and reports.
Ability to communicate effectively orally and in writing with people of varied social, economic, cultural, and educational backgrounds.

Ability to work effectively, tactfully, and courteously with the public and other governmental organizations.
Requires bachelor's degree in social work from an accredited school of social work and no work experience; or a bachelor's degree in a human services field from an accredited college or university and one year of directly related experience; or a bachelor's degree from an accredited college or university and two years of directly related experience.
An equivalent combination of education and experience may be considered.

Responsibilities

Assess, develop a care/service plan, coordinate, and monitor delivery of services to clients to assure the client's/and or family's health, safety, stabilization, and wellbeing in the home.
Provide or facilitate services to assist adults receiving placement services to adjust to their placement or independent settings.
Coordination with physicians, nurses, and home health providers to develop client care plans.
Provides intake and case management services including but not limited to Adult Protective Services, Energy Services, Community Alternative Programs, and Representative Payee, Special Assistance In.
Adult Protective Services after-hours on-call rotating coverage.
Interviews clients and relatives to determine the scope of the problems and client's ability to deal with it, performing assessments to identify needs and obtaining medical, financial, and social history.
Develops service and/or treatment plans and monitors service delivery for clients and/or families to address issues of employment, vocational, medical, health, or behavioral issues.
Provides supportive/directive counseling to address problems such as adjustment to illness or disability, adult placement issues, financial concerns, and socialization.
Establishes eligibility for specific programs or service.
Prepares case summaries and reports for reviews, maintaining records, files, and confidentiality.
Reviews and approves billing documents for client services or supplies.
Ensures that the plan provides for safety and wellbeing and meets federal, state, and agency requirements.
Discusses gaps in services and ways and means to fill gaps with supervisor.
Conducts the required number of contacts and/or home visits each month according to specific program requirements to assess the current situation and assure that services are being provided.
Explores alternative community programs, services, and agencies; makes referrals.
Provides supportive and/or budget counseling and education to clients, care givers, and families.
Provides a wide variety of services to alleviate crisis situations, enable families to be self-sufficient, and enable adults to remain in their homes.
Drives to homes, schools, health facilities, and other service agencies.
May transport clients.
Operates computer terminal entering and retrieving data from the appropriate state and county system.
Operates computer, scanner, calculator, and other standard office equipment incident to maintaining records.
