About The Position

The Division of Social Services at Cincinnati Children's understands that managing illness or hospitalization is stressful. Clinical social workers are integral members of every medical team, providing ongoing support and counseling, crisis intervention, advocacy, education, and connections to community resources. Clinical social workers are licensed professionals with specialized expertise who are guided by core values promoting self-determination, dignity, respect for all beliefs and cultures, and supporting patient and family strengths in their pursuit of health and wellbeing. The Social Service Division has over 150 clinical social workers serving patients and families through inpatient hospitalizations and outpatient settings. They assist patients and families coping with medical and/or psychiatric conditions, providing support for emotional, social, and financial well-being, and navigating the complex medical system. Services include emotional support and counseling, family issues/conflicts, safety concerns, discharge coordination, and access to community resources.

Requirements

  • Master’s degree in Social Work (MSW) required.
  • Licensed Social Worker (LSW) in the State of Ohio required.

Responsibilities

  • Performs psychosocial assessments considering family, social, and medical history. Maintains current knowledge of medical and mental health diagnoses. Reports suspicions of abuse and neglect to legally mandated authorities. Completes documentation with accuracy and clarity.
  • Collaborates with the healthcare team in developing treatment plans and identifying appropriate resources to support interventions. Works with patients/families/caregivers to identify appropriate resources. Maintains fluent knowledge of internal and external resources. Provides real-time, accurate information on eligible resources. Provides condition-specific and related medical, financial, educational, and social supportive resource information. Evaluates the effectiveness of clinical interventions and makes changes as needed.
  • Provides psychosocial services as identified in the patient's comprehensive plan of care. Offers supportive counseling to improve emotional coping and adherence to the treatment plan. Educates patients/families/caregivers and healthcare team members about treatment options. Provides self-management support to high-risk/complex patients/families to enhance their skills and confidence in managing chronic care conditions at home. Provides groups and programs for special populations as needed. Offers anticipatory guidance and planning to reduce or eliminate psychosocial barriers to discharge. Coordinates and facilitates family-centered care team meetings. Provides crisis assessment and interventions to restore equilibrium. Collaborates with referral sources and healthcare team members.
  • Works for system improvement, promoting patient/family/caregiver well-being. Identifies and advocates for patient/family/caregiver needs within the healthcare system and community. Acts as a liaison between the patient/family/caregiver, the medical team, and outside agencies. Mediates within the medical team on behalf of the patient/family/caregiver. Empowers patients/families/caregivers to problem-solve by exploring available options and alternative plans when necessary. Facilitates communication and coordination among healthcare team members while involving the patient/family/caregiver in decision-making to minimize service fragmentation.
  • Partners with the patient, family, and healthcare team in the safe transition of care to the next most appropriate level. Educates healthcare professionals and/or students on the psychosocial aspects of patient care and the role of social work. Promotes a positive work environment. Participates in professional growth opportunities. Attends divisional, team, institution meetings/conferences, and in-service programs to develop skills.
