About The Position
Mercy Medical Center is looking for a Licensed Discharge Planner / Social Worker (LSW /LCSW), to join our Case Management Team. The Discharge Planner is a licensed medical social worker who works collaboratively with patients, families and the medical team to ensure patients recover and achieve optimal well-being.
Requirements
- Bachelor's degree in social work required.
- Valid social work license from the Commonwealth of Massachusetts Required, minimally at the LSW level Required.
- Two (2) years of related professional experience required. Fieldwork placements may be substituted in place of one (1) year of experience.
- Must have extensive knowledge of community-based resources and programs, managed care, insurance authorization processes, medical terminology, and psychosocial aspects of illness and health.

Responsibilities
- Responsible for performing psychosocial assessments to determine patient care needs
- Provide ongoing case management support to the patients.
- Helps connect patients to resources within the community.
- Vital in assisting and supporting patients who will require discharge planning services.
